Yes, the State of Illinois Department of Aging has the Community Care Program available for you. We want you to continue to live independently in your OWN home, for as long as you can. Call Senior Services today at 1-800-283-4070 and make an appointment for a State Certified Care Coordinator to come out and speak with you about the Community Care Program.
